Enrollment for anyone covered under the collective bargaining agreement providing for participation in the plan is a snap. Take a moment to review your fund selections and how much you want to contribute per paycheck. It's as easy as these 5 simple steps:
Step 1. Please read your enrollment kit or review the L{earn} Online section of this site. Also, it's important that you review the fund prospectus(es).
Step 2. Download and fill out the enrollment form. A copy is also available in your enrollment kit.
Step 3. Choose the amount you want to contribute from each paycheck and how you want this amount allocated among the funds you've chosen.
Step 4. Fill out the beneficiary form (available online).
Step 5. Return the completed enrollment and beneficiary forms to your employer and save a copy for yourself.
That's it! Your participation in the Plan begins with your first payroll deduction from your next paycheck.
